Enrollment Packets for Participants

The Individual Enrollment Packet is available for download for applicable RecoveryLink peer programs. All participants are required to review the documents and acknowledge willingness to engage in the peer program and with a peer recovery support specialist. 

The enrollment packet includes:

• Peer Services Agreement
• Declaration of Confidentiality
• Grievance Policy
• Individual Grievance Form
• Consent and Authorizing Disclosure of Confidential SUD Patient Records

Managers at organizations using the RecoveryLink dashboard application will inform Peers of enrollment document requirements and provide detailed instructions for completion and secure file storage.
