Initial Recovery Plan
Initial recovery plans are completed following a full intake with every participant who has selected the comprehensive services pathway. Recovery plans are intended to be created in partnership with a participant and may be entered into RecoveryLink while in session, or following a session but on the same day.
Recovery plans require information related to complimentary recovery plans, a participants hope and long-term goals for the future, and a list of supportive people (including relationship to the participant, if it is okay to contact the person, and their contact info if so).
The goals of a recovery plan are all optional and include selecting the recovery domain, setting goal status (active, inactive, complete, or in the future) and goal priority level (high, medium, or low), applying SMART goals and action steps (i.e., tasks) to achieve the goal, as well as any general notes pertaining to the goal. Optional domains are: 1) Housing, 2) Employment, 3) Education, 4) Physical Health, 5) Mental Health, 6) Safety, 7) Daily Living and Routine, 8) Legal, 9) Relationships, 10) Finances, 11) Spirituality, and 12) Other.
Referrals associated with a recovery plan goal, or general referrals requested by the participant, can also be created at the same time as an initial recovery plan or during a follow-up engagement, in a brief check-in session, or ad hoc through a participant profile. Referrals have status options of (Drop-in referral, active, inactive, and complete) and can be updated from the participant profile to show progress towards engaging with referrals over time.
The most up to date version of the recovery plan can be viewed through the participant profile recovery plan tab. In addition, if an ad hoc update or revision to the recovery plan is needed, peer specialists can use the "update recovery plan" button on this tab to update the recovery plan without need of completing a formal engagement session.
